Is the "request rate plan or offer change during return period link not working?

When trying to access the form the following error message shows up:

@JP Serrano this is an old form used during the Black Friday / Boxing Day sales period.
If you need to change your plan due to a sales change during your return period now, you will need to request a callback via Koodo
