Is visual voicemail already included in the plan or is it an add-on? If it is an add-on, how do I request it and what is the price? I am using an iPhone so visual voicemail is very important for me.

Visual voicemail is available as a separate add-on for $5 a month. It is not included in the standard plan.
Assuming you are on a postpaid plan, you can add it by signing into your self serve account, manage plan and and add-ons->manage add-ons and selecting the visual voicemail add-on.
