I signed up for Stream+ Basic, when I launch Disney+ Standard with ads though Koodo to enable Disney+ it asking me to choose a plan that looks like I need to pay $8.99 on Disney+ website. I used a Private/Incognito window with same results, Brave Browser on my phone with adblock turned off does the same thing. Netflix Standard with ads & prime worked.

If you have stream+ basic, Disney +Standard with ads should be included. To confirm, when you go to activate, it’s saying you need to pay 8.99 extra to the 23.99?

it says "complete subscription" on Disney+, and it shows plans from $16.99, $15.99 and $8.99, if I want crave or TSN or both tap on the tabs, all for $41.75/mo

There should be an option to ignore any upgrades they’re inviting you to pay extra for.

You would need to click on the link Koodo provided. Can you log out complete of all Disney+ that you had and clear cookies of the web browser? As well, can you activate with a different email address?
