How do I request a refund for an option which i did not request?

I noticed that my bill for this month was $6 more than usual.  I logged checked the charges and the LD option of $3 for 2 phones on my account was added.  I did not request this option to be added to my account.  How do I request a refund?

There was a price increase for the add-on in March. If you checked your previous bill, you will see it in small print on the last page. If you do not want the add-on, you can remove it on your Self-Serve account.
