"Exclusive Offer" not available to all plan members

Why is the $45 for 20GB only available to 2 of the 5 members on my account?  I would like to switch everyone over to this plan.

Specifically, all 3 of my children are on the same plan, but only 2 were offered the above “exclusive offer”.

This offer is also far better than the plan my wife and I are on.  As the account owner, should I not be able to move all members to the new plan offer?

Usually personalized plan offers are just that - personalized. You COULD try to contact Koodo and see if you can get the offer on all your lines, but that’s not a given at all.
