I pay $5 extra a month for visual voice mail. Yet for the last several months I have received the error message: unable to access visual voicemail. It was implemented and it worked and now it doesn’t. I am still paying the charge on my bill for this service I haven’t received for a while. Do I need an app? Is this gatekept by the make of my phone? If it’s just the fact my phone is too old I’ll unsubscribe.

Koodo visual voicemail didn't require any additional app. If it worked in the past, there might be some update in your phone that might have created the problem.
Have you tried to reset network settings of the phone? You could also try to remove the addon for one day then add it back to see if it can trigger the visual voicemail configuration.
