On the included features, it indicates “Unlimited Canada-US messaging”, which means that international SMS/MMS is not included (other than canada-US).
One of the perks available is listed to be “Unlimited international SMS (from Canada).
Can someone clarify whether this “perk” includes sending international SMS/MMS while in the USA? Otherwise it doesn’t make sense.

That is correct. It seems to suggest that you can have unlimited international messaging from Canada if you decide to choose the Perk. However, when you are roaming in the U.S., it will only be limited to the two countries.
If you get the international messaging perk, my interpretation of the wording would be that it would carry over into the US as well
