I already left left the Koodo and my contract finished and my tab is paid off but I realized my iPhone remained locked. I have no access to my Koodo profile anymore, how can I unlock the phone?

When did you buy the iPhone? if it was after Dec 2017, that iPhone was unlocked already.
If you bought it before Dec2017, you would have to contact Koodo directly and ask a rep to unlock it for you as you are no longer a Koodo customer.
