My uncle recently passed away and my cousin is paying his bills and his phone was through Koodo. She still doesn’t have access to his bank accounts but can get a cheque made at the bank to pay the bill. Where do we send the cheque? What is the billing address for Koodo?

They key thing is you need the account number to pay.
It might be easier to deal with if you go through the process to inform Koodo of the passing and to cancel or complete a transfer of ownership. Instructions are here:
