How does Stream + access internet? Does it run off my phone? How does it work if I want to watch it on my TV ? Is it connecting through my Apple box? Or do I need a separate internet provider to use the service?

It is just subscription just like when you sign up with them directly. So you can use it in any way you want and with any device/provider.
