I added stream plus but there is nothing for activating Amazon. All the others showed up ie Netflix etc. but nothing for Amazon. How do I activate it?

Amazon subscription is another add-on. It wasn't included in the streamplus add-on . If you want it, you can add it in your self-serve >> plan >> add-on

And if someone remove and readd, would the person have to reactivate/relink all other service again?

The new version is subscription based (similar to Koodo Internet) and it includes Amazon Prime, Netflix and Disney+.
Yes, the services need to be relinked once new Stream+ is purchased: https://www.koodomobile.com/en/help/stream-plus
