I accepted the free Apple TV+ subscription several days ago, but I have not yet received the activation email. Why is this?

That was strange, have you checked if you received anything in Junk/Spam folder? As well, how did you accept the offer? Was there any confirmation either by text or email at that time?
When you login to your self serve can you go to the Subscription area and see the Apple+ TV icon. There should be an activate button next to it
