Porting old number

Having trouble with porting my old cell number. I have entered all of the correct information form my old provider but it still gives me an error.

What's the error say? Try using only your old provider's account # as that's a sure fire way to make a port request. The other stuff may make it fail. Also, check to ensure it can be ported here: https://secure.koodomobile.com/checktransfereligibilityparrot/default.do?lang=en&appname=otherportal If it says not eligible, then you have to pick a new #. If it says it's eligible but doesn't want to go through, call Koodo at 1-866-995-6636 and request to speak to the porting team.
It says that it is eligible, asks me to enter all of the details from previous provider (account name, number, PIN# etc). I do that, then it gives me an error saying "Failed Data Format Validation".
