I can't make it past Step 4 on Koodo Prepaid Online Activation....HELP


I enter all of the *required fields and click continue and the page refreshes without my passwords in the required field. How can I resolve this issue to continue on to step 5.

The problem seemed to be my email address, which I would of used to register my last koodo phone. I wish there was a troubleshooting message that let me know what was wrong so I didn't waste time re-entering my information every time
