Cannot unlock phone online, cannot receive calls (can't get support callback)

Good call making it so your customers can't reach anyone by phone. It's not like you're a telecommunications company or anything. Nearest Koodo kiosk is 5 km away.

Have you tried to setup a callback through their digital assistant www.koodo.com/chat Below is my chat with it. Just ask it to unlock your phone. Option for callback will show up.

If you can't receive calls you can also send them a message on Facebook or Twitter asking them to unlock your phone. Have the IMEI number of the phone handy to give to them.

Was your phone purchased after December 2017? If so it is already unlocked, and you need not call.
