I have recently transferred from Telus and did not receive a customer service agreement (physical copy or via email) and know I need to create a service account but don’t have an account number etc... how do I get this information? should I call telus or Koodo? the automated Koodo chat was no help. thanks

You should have received an email with this in it and an email to setup your self serve account. Check your junk folder as well to see if it ended up in there.
If you didn't receive any emaisl then yes, you'll have to get in touch with customer service. Easiest way would be a pm to koodo via Facebook.
