$5 rebate missing from bill

I signed up for a $50 plan with a rebate for $5 every month for 12 months (making my plan $45).  I got my sim card mailed and ported my number. I have gotten my first bill and the rebate is missing.

I’ve been trying to get a call back from a rep, but each time i try, it says there are no time slots.  I can’t find anything useful in the selfserve menus.

Can someone tell me how to fix this so i get my rebate?
