Weird Call Forward Icon issue?

I have an unlocked Galaxy II. I call forwarded my phone to a friend in Calgary who would handle things while I was away. I removed the SIM, drove to the USA, bought a T-Mobile SIM and had a nice vacation. When I returned I put my Koodo SIM back in and hit #21# . Calls appear to come through and all is fine EXCEPT I still have the call forward indicator at the top of my phone (picture of an old style phone with a right arrow). And the phone says "Forwarding Video Calls" when it boots up. I re-forwarded and re-did the #21#, rebooted, yanked and reinstalled the SIM. Pulled the battery and put it back in. Can't think of anything else to do. I don't do any video calls but what's up with the weird call forwarding icon? Thanks in advance for any help.
