Call customer service and ask them for the code. As of right now, they're the only ones who can give it to you. It's been suggested that PUK codes be available in Self Serve, which is an excellent idea. I've also suggested they be written on the plastic cards that SIMs come packaged in, which I saw on a Columbian SIM card one time and thought it was genius. I've also suggested it be placed on customer agreements.
