Porting " Missing Required Field" error: can't get it working


I've got an account with koodo and bell. I want to port in my bell number. I pushed all the information and I always get the same error " Missing Required Field" I did put my IMEI (galaxys4) and my account number (9 numbers) and my pin number from bell. I did input an additionnal phone number. What am I missing?

There's an alternative account number at bell for "online and telebanking" which adds numbers and letters to my account number. I tried it and it didn't work.

That I'm not sure on. Maybe give Koodo a call and have them try.
