Hey, I have a really great older plan (unlimited Canada wide talk and text, 3 GB data for $60/month) that given the current plans available I have no reason to change. I have been accumulating a positive tab balance for a while now and am getting close to $150 (which as I understand it is the maximum).

My question is, if I choose to use that positive balance towards a new phone and go back to a negative Tab balance (say for example I purchase an iPhone 5s, use the positive Tab balance and put the remaining $210 on the Tab), will Koodo still contribute 15% of my bill towards that Tab balance? What if I just use the positive Tab balance and pay the rest of the upgrade up front, will the 15% contribution continue then towards a positive Tab balance again?

Once you use your Tab you will be on the new tab and there will be no more 15% contribution

So even if I don't put any portion of the upgrade on the Tab to be paid back to Koodo and just use the positive Tab balance against the retail price of a Koodo phone the 15% contributions from Koodo will stop?

That is correct it will stop. You will automatically be moved to the new tab system once you go ahead with it.
