Rather than call, where you might be in for a long wait to speak to a rep, send Koodo a private message via Facebook or a direct message via Twitter. If you prefer to call, ignore the $10 voice prompts and politely explain to the rep the problem. Errors on billing statements cannot be corrected by you, therefore there is no charge for the call.
@ Clayton, there was a phone number change during your upgrade and back again. I believe you are aware of the reason, based on the notes on your account. This changed likely triggered another tab charge although all it means that you have less to pay back on your phone now. You can see it if you log into your account and look at your tab transactions.
