Go to: Settings > Call > Additional settings > Fixed dialing numbers > Change PIN2. The default PIN2 code is 0000, unless you've entered a different PIN2 code. If the wrong PIN2 code is entered three times in a row, the PIN Lock will be automatically activated.

If you enter another incorrect PIN2 code and it locks the SIM, you will have to call Koodo from a landline or friends phone and get the PUK2 code from them. This will allow you to reset the PIN2 code. You could try entering the last four digits of your phone number if you haven't already tried that.

It should be 1234. If your SIM card gets locked, you will need to call Koodo customer service and get the PUK2 code for your SIM card. If you enter your PUK1 or PUK2 wrong ten times, you will need to buy a new SIM card, so don't try to guess that one.
