If I want to keep my existing phone (iphone 5s) but upgrade to a more expensive plan, is that technically considered an "upgrade" by Koodo? My tab will be paid off.

Activate a new line/account or upgrade your phone on the Tab qualified to get a new promo plan :)

Mayumi, are you saying that by keeping my existing phone I cannot upgrade to a new more expensive plan and take advantage of the special offer that ends Sept 30 on the medium plan?

I should say that you can upgrade your plan to a promo plan(+1GB) if you paid off your current tab.
But bonus "1000 International LD minutes" are only available for new activation or upgrade your phone.

So when Koodo uses the term "upgrade" a phone, does that just mean change to a different phone or what? How do they determine if the new phone is an upgrade?

If you buy a new phone from Koodo, then it's an upgrade.

How do they determine if the new phone is an upgrade?
I expect there is a presumption that folks wouldn't purchase a phone inferior to the one they currently have, though I guess someone might if the deal was sufficiently lucrative.
