I currently have an iPhone 4 8GB on what's now called 'tab small' but at the time was just the tab. My current tab balance is -122.55. I want to upgrade to an iPhone 5S on the tab medium, so I have a few questions.
1 - How much would it cost to upgrade. I know that I have an outstanding balance so how would that effect the 300$ credit on tab medium?
2 - Can I keep my current plan. At the moment I have a 45$ plan with 1GB of data that was a special at the time I got it and no longer exists, so I was wondering if I can keep this when I upgrade.

Retail Price of iPhone 5s is $725
You can put up to $300 on your Tab M so you can use your available tab $177.45 (($300 - $122.55) to upgrade your phone
You pay $547.55 ($725 - $177.45) plus tax at the store and your tab will be set to negative - $300
There will be $5/month tab charge

And yes,you can keep your $45 plan.

1) You would take the difference. Since you have $122.55 owing, moving up to the Tab M to $300, you would have $177.45 to use toward the iPhone 5s. That means you will pay $725-177.45 = $547.55 for the iphone 5s and have a $300 tab M
2) Yes you can keep your current $45 plan. To qualify for a Tab M you need a minimum $30 plan
