Do you have a Koodo account already?
If you do, this link should help you :

If you followed the instruction and still didn't work, call Koodo from different phone at 1-866-995-6636 option 6 for tech support ( or press 0 few times)

Tell them you tried reprogramming yourself - followed instruction-, but it didn't work. They will help you out :)

I know It will be shut down soon.
I haven't see the official date for it, but I heard it will happen at the end of 2014 or sometime in 2015.

It may be available to use if you still have an active device but if the device isn't active i don't think there is a way to activate any longer, again calling Koodo will get you all the answers *611 from any Koodo phone or 18669956636 for any non Koodo device

@Cameron. Oh, I see what you mean :)
@Cody. Let us know if you could activate your phone through tech support (if you don't mind :) )

You will definitely need Koodo's help in activating this (re: CDMA, no sim). Let us know if you run into any difficulties, good luck!
