US Roaming add on not working

I have an iPhone 5 and purchased an add on for use while in the US and it is not working.

I have tried resetting my network connections and this does not resolve the issue.

Under the cellular data tab the only completed field is APN which reads sp.koodo.com, all the user name as password fields are blank,

How many bars of signal do you have?
Does it say which network you are connected to?

Nothing was working.

I ended up speaking to a Koodo agent (actually 3 of them), we finally diagnosed there was a software update for my iPhone, once I installed that and turned the 'Automatic' feature under Carrier in Settings everything worked.
