Has the SIM card been activated with your account? You must register the SIM with Koodo on your account.

Can you test the sim in another Koodo phone? The sim could have failed

For android:

Go to settings - wireless and networks - more - mobile networks

Make sure "use 2g only" or "use gsm only" are disabled (not checked) as koodo doesn't support that mode.

For BlackBerry 10:

Settings - network connections - mobile network - network technology should be anything besides just GSM.

For iPhone:
Settings > General > Networks
Make sure 3G is on

That'd be a pretty old iphone. Newer ones have it under settings->general->cellular(ios6), or just settings->cellular (ios7)
