Unable to Register for self serve

i got a new phone and the account transfered from my mom/my name to just my name. when i put in my name and phone number using the app it goes on to ask me my postal code and account number.  after i put those 2 details in it says that "information does not match with the account info" even though its the correct postal and account number on my form.

Have you tried signing up directly through the website, rather than the app?

i've tried to access it through the link sent in the e-mail on my desktop and both my ipod and cell phone using both my home connection and my wifi at work. every time i hit the "Register now" button i get a "DNS address cannot be found" or "DNS_PROBE_FINISHED_NXDOMAIN". i've tried  running "netsh winshock reset catalog" and runing my computer along with manualing setting my DNS to 8.8.8.8 and 8.8.4.4 but nothing seems to do the trick when trying to access it.
