Unable to get a Large Tab due to a problem with the Credit Check

I know I have a pretty solid credit score, and have never had problems opening up any accounts.  I was pretty surprised with this result.  I even verified my Credit Score afterwards and it's definitely in the Good Rating.  While I do have a joint mortgage with my partner, I do not have any other debt beyond that and consistently pay my bills on time, etc.

I suspect there was a problem with info used to look up the credit (as the names on my IDs are not consistent).  Question I have is: Was there a hard check done if Koodo told me that they couldn't give me a Large Tab, but only a Small or Medium tab?  I'd like to try it again but I'm afraid of negatively impacting my credit score with these attempts.

Was there a hard credit check done?
