Tab overcharge

I've had a plan with a $5 tab charge for over a year and so my tab balance has been at $0 for a few months now and my Tab Type is No Tab now but on my bills I am still being charged a $5 tab. At first I assumed the $5 would be added to my tab balance and I could use the money for a new phone but my tab balance has remained at $0. 
How can I remove the charge from my monthy bill and find out where my money is disappearing to?
