If you are upgrading your phone then there should be no charge for a new sim due to the fact of the compatibility of the device.
If you are just buying a new sim then, yes. the cost would be $10 plus tax

I've upgraded my phone and needed to pay an extra 10$ for the nano-SIM card...

You actually bought the phone from Koodo?

Yep, but it doesn't matter! I can't say 100% sure I got charged. But more like 75%

That is very interesting. One would think that since you are upgrading, in other words, making Koodo aware that you plan to stay, an adjustment could be made. BUT, that is a judgement call for the agent(s) involved, to be honest

I needed a micro sim for my loaner phone and I had to buy one at a Kiosk. 10$

woah...that was a situation beyond your control. personally, i dont think you should've had to pay anything. But, the technicality is that a loaner phone is an option, not mandatory

If you upgrade in-store and actually purchase a phone from Koodo, there is no charge for a sim card. If you are buying a phone from a 3rd party, that's not upgrading and you will have to pay $10 for another sim card.

If you mean does it cost to change your service over to another sized sim the answer is no but you will need to buy the new sim.
