Did you order your phone through online store and you just received it? If so, you should've received an Email about how to activate your SIM. Also, there should be a card/letter i the box that explain how to activate it.
Very strange to send your ID's through fax. Were you advised of this at the kiosk? Were you trying to activate a plan outside of the province where the Koodo store is located? Are you around the age of 18/19? Just trying to think of possible reasons why they'd want to see your ID via fax.
