To my knowledge I heard of phones doing this when they put a non Koodo sim card in the phone. Also, if maybe there might be an issue with the sim card as well. Try moving the battery and sim and plug back in again after 30 seconds.

This could also be an issue with the phone itself, if Rocky's suggestions don't work trying factory resetting your software.
