Phone network suddenly locked

Hi, so I have this unlocked old Samsung Captivate (100% it's unlocked because it went from Rogers to Bell to Telus during its lifetime) and since last Saturday I don't get to access the Koodo network anymore (I always payed my bills). Although I'm receiving a signal (-83dBm/15asu on UMTS:3 network) I don't have access to any services from Koodo even though I did 2 days ago! Is it the sim card that's acting up or am I forced to buy a new cell/sim & forced to switch providers? ANy help would be appreciated! Thanks!

It's a bit hard to troubleshoot you without having access to your account and/or phone, but is it possible for you to try the SIM card in another phone? Then you'll know whether it is the SIM card acting up, or perhaps the phone itself :)
You can check in your self service if your services got suspended, though from what you're saying, that sounds highly unlikely. I suspect a hardware problem!

While the phone is on try taking out the SIM Card and battery for a minute or so then replace them and turn the phone back on. Let us know if this helps.
Oh also make sure the phone didn't get set to 2g only as koodo doesn't use this only Rogers does. Also make sure airplane mode isn't on.

I made sure to have 2G unchecked, Data enabled, Airplane Mode off since I could go on the internet via wifi & took out sim & battery & restarted. I also flashed my phone & factory reset it to no solution. I haven't had the possibility to check the sim functionality with another device due to the lack of the latter. But if it is a problem with the sim, will Koodo replace it free of charge? Thanks!

You said you flashed your phone which could very well be the issue if your phone was unlocked by rooting and not an actual unlock code entered to unlock the network setting..
Is it telling you the network is locked when you first turn it on and asking you to input a network unlock code?

No nothing like that. It just says Network Locked with no notifications for an unlock code. The Network was locked even before I attempted to flash to maybe fix it. I'm starting to think that it's the SIM that's the problem.
