Paid off tab in full, why does my bill show I was double charged a monthly tab rate?

I recently switched over from a monthly plan to a prepaid plan. Before switching over, I cleared my tab in full. It said the amount would appear on my final bill.

I just received my final bill and it is showing two tab charges: the payment in full ($422) and the monthly large tab ($21). Seems to me that the monthly charge of $21 is in error, since I have already requested to pay off the tab in full. 

I prefer to NOT call the Koodo hotline, because I really don't want to wait an hour on hold to do this! I don't want to use up my prepaid minutes to correct Koodo's error.

Can someone contact me to clear this up and possibly reverse the charge? 
I will also send a private email, but I want to check in here about it as well.

You might have to contact Koodo directly as they might need to validate your account. You don't want to post your personal information in this public forum.

I would suggest you send message to their facebook, as you don't have to wait on phone.
