You need to send Koodo a private message to their FB page and include the IMEI # of the phone and they will get you the unlock code

Note that once you get the unlock code, to get to the unlock screen you will need to insert a non koodo/telus sim in to the phone

You can also try unlocking via self serve.

self serve recognizes my email , it says to wait 15 minutes to get the email, but i never receive the email to be able to log in probably because my account is too old from 2012-2013

If you don't have a different provider SIM Card to put in to bring up the unlock code input you could try this I just found it online so never tried personally, so if you do try and it works feel free to come back and let us know if it worked or not to help others, thanks.

Samsung Galaxy S2, S3, Note , Note 2)

Follow these step carefully, as long as you have the correct code it will work 100% (Your phone must be updated to the latest firmware!)
1. Please trouble shoot as usual with the customer, the code must be correct and the phone MUST prompt for the code.
2. If nothing we can proceed to use the "release method" on the phone(s):
a. Go to the Dial pad and dial *#197328640#
b. Please click on number [1] UMTS > [1] Debugging screen > [8] Phone Control > [6] NETWORK LOCK
c. You will see 3 options
d. Please click on number [1] Perso Sha256 INFO, the screen will change slightly
e. Please hit the MENU button then when the menu pops up please carefully hit BACK to return to the previous screen
f. Please click on number [2] Perso Sha256 ON, the screen will change again to a one line message
g. Restart the phone on the screen by holding the power button and click RESTART
h. When the phone prompts for the unlock code simply enter the unlock code
i. Your phone is now permanently unlocked

The *# code looks very familiar. I used it to successfully unlock my Galaxy Ace running Android version 2.3 (Gingerbread). The same can't be said for my Galaxy S2x that ran Android version 4.1.2. The *# code had been patched over and/or components were removed from the menu on later Android versions from 4.1 (Jellybean), onward.

If the firmware on Zoltan's Note 2 is up-to-date, the last firmware upgrade was Android version 4.4.2 (KitKat). Give it a shot, but I have my doubts the *# code method will work.
