Are you sure you need the PUK code or unlock code for the phone (because of the phone was locked to anther carrier)?
If you need the PUK code, you would need to contact Koodo directly (messaging to their facebook will avoid the wait on phone).

I know you're asking for a puk code which has nothing to do with unlocking your phone from another carrier.

What phone was it? and what button did you push that made it unusable? Normally  if it is locked you might just need a correct passcode to enter.
