To answer the first question, someone probably was using your phone and locked it for some reason if you did not set up a lock yourself.
Second question, the password would be incorrect which is why the phone is not accepting it

What type of phone do you have.

If you didn't set the password and do not have access to it you need to erase the phone in order to clear the password. You will loose everything that isn't already backed up.

Is it asking for a password or something else like a PUK code just to make sure we give you the right information.

Blackberry Curve

I wrote down my password when I got the phone but now it seems to be incorrect. It is the password they are asking for

Also be careful, after 10 wrong attempts Blackberries generally erase themselves (for security reasons), so while you solve one problem (you don't have to worry about that password anymore) you do loose everything on the phone.

try default code like 0000 or 1234 to reset it .If it doesn't reset your mobile then visit http://www.unlocking4free.com here they provide master reset code for some Nokia DCT phone and Alcatel old mobile models at free of cost using this code you can reset your mobile and again set a new password.
