If you have an Android phone try this Android Voice Commands Fix Tip
1.Choose “Menu” and then “Settings”
2.Choose “Applications” then “Manage Applications”
3.Find a way to “Show All” your Apps in this “Manage Applications area”
4.Scroll to find the App called “Voice Search” and Select it
5.Press the “Clear Data” button (yes, do it)
6.Go Home
7.Go to the Market and Search for the “Voice Search” App by Google
8.Select the Free “Voice Search” App and Download it or Update it to your phone
9.If asked, say “Yes”, you are OK letting Google watch your voice commands corrections

just to add one step to the instructions mentioned above
under step number 5 before clearing data , you have to stop the app by pressing "Force Stop"

There are two versions of Google Voice Search that basically perform the same function. The first, comes bundled with ICS and Jellybean OS versions on select devices and can be identified by a blue icon with a white microphone.

Pressing and holding the Search button (magnifying glass) will invoke Google Voice Search, which has a white background with a black microphone that turns red when tapped and remains red while issuing voice commands.

The second, is a standalone version presumably for lower-end devices that don't have the Search (or magnifying glass) button along the bottom edge of the device. (i.e. Galaxy Ace, Galaxy Ace 2x).

The app is identified by an icon which is light grey in colour, with a black microphone. The Google Voice Search interface for this version, has a black background with a red ring and white microphone in the centre. In some applications, an audio waveform can be seen when issuing commands.

For both versions, I wasn't able to remove or re-install the app. Voice Search is a system app. Google Play knows when the app is installed/disabled. The only way I could remove the app was to run No Bloat, (device needs root privileges) delete the app, reboot and then I was able to re-install the Google Voice Search app.

A good file manager and a rooted device easily found the string of text of com.google.android.voicesearch. This is why you wont find the actual app name Voice Search in the Applications manager listing, unless you're running the standalone version.

The standalone version also uses the same string of text com.google.android.voicesearch as its file identifier and can be removed in the same manner.

These tests were performed on a Galaxy S2x (JB 4.1.2.) and Galaxy Ace 2x (ICS 4.0.4.).

I have heard that Samsung is aware of this as it is a global issue right now.

I found the "com.google.android.voicesearch" that Rikkster mentioned. I'm wondering if there is any way to disable the google voice search app completely. I tend to hit it quite often and I don't ever use it.

Android Commander may be an option, the "com.google.android.voicesearch" file is stored in the data directory: /data/data/com.google.android.voicesearch. A rooted device is required to access system files though.

Root Explorer is what I use.

With so many people experiencing the same error I'm just going to wait and see if there is a fix and I'm not going to bother changing any settings yet.
