I purchased a few months ago the LG G5 from the Online Koodo Store. Is this phone factory unlocked (i.e. can I use it on another carrier in the US or Europe like I was doing my my Nexus 5)?

No. You will need to get it unlocked.
seriously..... I was sure it was unlocked. is there a way to check it without actually buying a Sim card? i.e. in the settings?

The easiest and most reliable way is to borrow a SIM card from someone you know to see if the phone tells you that it's locked.

I assume that the sim must be from another carrier ?

I think a have a old T-Mobile SIM from the US. It is a micro-sim and not a nano-sim... I think I can just cut it?

Yes, you could cut the T-Mobile sim card. Btw, you can go to a local cellphone retailer (anything other than a Koodo/Telus store) and ask to borrow a sim card from them (they might have a spare, or might be able to test with their sim card).

LG unlock codes on ebay for less than $5. It's not expensive to unlock your phone.

At the time of writing, there's no way that you'd get a carrier-unlocked phone from Koodo unless it's the Google phone (Pixel/Nexus). Carriers carry carrier-locked phones hoping that those who use these phones would use it on their network (rather than switch the service to a competitor). If carriers were forced to carry only unlocked phones (like in many other countries), you wouldn't benefit from the awesome discounts and promos that carriers have here. I rather unlock an Android for $5 on eBay than pay $100-300 more for a phone (whether it's up-front or over 24 months). Just my 2c.

I can confirm that it is not unlocked. I purchased the G5 from Koodo, and then unlocked it via eBay for about $5.

Try opening the dialer and entering: > #7465625*638#
That might show the unlock status. Or it might do nothing at all.

just tried it. didn't do anything...

Dial 2945#*831# (Assuming your LGG5's model number is LG-H831). If not replace "831" with your model number. This will bring you to the unlock screen. On my LG G5 purchased from Koodo only the sim card is locked, but the phone appears to be unlocked.

Skip all the "hints" and follow titaniumtux's advice. The only thing that matters is when you install a "foreign" SIM. It either works or it doesn't, but probably not if bought from Koodo. Unlocked phones are usually prominently advertised as such since they carry a price premium. Never seen a carrier use this as a selling point.
So I need another carrier SIM card, when inserted it should ask me for the code? Let's say I buy one of those codes on eBay, and that code works, it will stay unlock for all others SIM cards?

Correct, although you should save the code, as occasionally phones will re-lock themselves if the OS updates to a new version. It's rate, but I have heard of it happening.

Unlock code is like $4-$5 on eBay. Its not like iPhone that you have to pay more than 50$ just to unlock.
