Is the$45.00 (500 Data, 500 voice) still available or has it been phased out?

When looking at different plans, I saw something which led me to believe if I changed plans, I would be unable to go back to the $45.00 plan...is this correct?

If your current plan is no longer available (not advertised on the Koodo website), then you cannot switch back to it if you switch your plan.

ok, it was still advertised on the Koodo website..but when I was about to change, an !mark popped up and said your plan is no longer available, if you switch, you cannot get the same one, HOWEVER, there was no !mark beside the plan and it is still advertised..

The $45(500mb, 500 min) plan is still available.
I think the "!" marks popped up automatically to confirm your change. (so they just want to making sure that you cannot get your plan back if it'is no longer available after you switch)

Mayumi is right, the message is just a standard message whenever you change plans
