What are my upgrade options from iPhone 4s now my contract is expiring? Phone & plan?

You can choose any phone and any plan or choose any phone and keep your present plan. Keep in mind that your tab available to you is dependent upon which plan your have or choose.
You can get a small tab which takes $150 off the point of purchase price of a phone of your choosing with any plan.
A medium Tab takes $300 off the point of purchase price of a phone and requires you have a plan of $30 or more.
Your final tab option is a large tab which takes $500 off the point of sale price of a phone but requires you get a tab large plan.
You could also just pay full price for the phone of your choosing up front.
**With the new 2 year pay off guarantee your tab will be cleared in 24 months even if you haven't paid it off in full yet.
Just a tip but the best bang for your buck is a medium Tab IMO. Also try to stay away from getting a large tab / large tab plan.

Oh yes you are right. I genuinely never remember that because it wasn't something that was recommended and in fact not a lot of people got an iPhone on a 3 year.

So to keep my present plan I'm not automatically entitled to a free upgrade after 3 years?

No. No phone company works that like. You finish your term and then upgrade onto a new term with that specific pricing. Choose a Tab size (if you get a tab L, you need a Tab L plan) and pay for the price of the phone on that tab. For example, a 16GB iPhone 6 is $275 on Tab L

Actually Koodo converted all contracts to Tabs a while ago.

You are not locked in.
All subsidies are done through the Tab program now so you see how much you actually owe so there are no more unreasonable "penalty fees" carriers use to use to screw customers over
