If my phone breaks, or something happens to it, I agreed initially when I purchased my phone that I would pay a $99 activation fee, because I didn't want $5 a month added to my phone bill. I have changed my mind, and would like the $5 a month added to my phone bill, so that I never have to pay the $99 activation fee, if my phone breaks. How can I go about doing this?

You are gonna have to call Koodo.  I am unsure if you can switch.

However there is no mention about switching payment from monthly to upfront

Hey Stephen! Just to clarify, are you saying that you already purchased the $99 extended warranty and would now like to change it to pay for it monthly? If that is the case, it is unfortunately not possible.

I was only offered the $6 a month added to my bill. I was never offered an extended warranty by paying $99 upfront, by the customer service representative, or I would have purchased it.

I'm a little confused. You mention an activation fee, but we do not have activation fees. I could only assume that you were talking about the extended warranty. Would you mind elaborating a little more please?

Perhaps I'm a little confused, because what I am stating is what I recall the customer service representative stating. I cannot elaborate any further, because this is exactly as I remember them stating.

Okay, well if you did not purchase an extended warranty then your phone has a 1 year manufacturer warranty that is provided by Samsung. During that first year, you can send your device for repair from any of our Koodo kiosks http://koo.do/kdostore.
