I switched from a pm and while in store, representative lost the new activated sim and I got charged for it

Looking at my bill at it's a $10 extra charge for "equipment exchange". Sim was lost while representative was multi-tasking, and could not provide me with the activated sim to insert it in my phone. He got another, switched it over while on the phone with service rep from koodo. Why is this my fault ? The rep said he will take care of it but that never happened.
