You need to call Koodo and ask for your PUK code

Do not continue to enter the wrong code or the SIM will become permanently locked and unsable

You will need access to another phone from a friend or landline.

This is a security feature that was enabled to ensure your sim will need a code when you phone is restarted or inserted into another phone. It is not a Koodo function, but a standard used for all GSM networks/phones

In the future you can turn this off by:
Settings.> More > Security > Set up SIM card lock > Uncheck the box that says Lock SIM card.

An idea was put forward to give customers the PUK code but it does not seem to be planned by Koodo

Maybe someone can revive the idea Sherri?
