The $4.99 is an immediate a sign of an application purchase. Yours Sandra took place on July 14th. It looks like you called an 800 and/or 888 number that afternoon. If you download your invoice you'll see 3 calls. That number belongs to a company that charges this fee for their services. We have nothing to do with them. You could certainly dispute the charge by calling them as they state here:

"So if you are unsure as to why you are being charged, when the charges were made and/or where they originated we recommend you contact our customer support billing enquiries team online or call 1-866-340-4990 and select option 1."

At this point we can only speculate what the charge was for. Your best bet would be to get into contact with Koodo. They can tell you what the charge was for and if it was a mistake. Shoot them off a message via facebook or twitter.

I had the same happening to me. I called Triton Global but the lady on the phone was very rude and did not want to make any adjustments. I filed a complain with CRTC.
