What seems to be the exact issue?
If your trying to use the self serve app know that it has been shut down so it will not work anymore and you need to use the web based version of self serve

Go to www.koodomobile.com and tap the menu in the top left corner and select login

You can also set up a shortcut on your phones Homescreen, when your on the login page tap your browsers menu and select to add shortcut to Homescreen.

The number for customer service is 611 from any koodo phone, 1-866-995-6636 from any other line. Bear in mind, though, that they will ask you the exact same questions and will likely give you the exact same answers that we would be able to give you.

Are you on prepaid or postpaid? If your on prepaid and haven't topped up your base plan for over 90 days your account will be closed and you will need to get a new SIM Card and set up a new account.
If your on postpaid You'll need to call the number me and Timo gave above and talk to a customer service agent there.

If you are behind on your payments, you will need to make a full payment plus a reconnection fee in order to get service back again
