i cannot set up Self Serve because my email is already registered. How did that happen?

Likely because the shop set up your account with your email, and subsequently , having missed seeing the confirmatory email sent you, you set up a second account. You’ll need to contact Koodo to have them delete the newest account,and send you a fresh confirmation.

You can contact Koodo by PM from their Facebook page, DM through Twitter, or calling *611 from your phone. Facebook and Twitter consume less of your time.

You should have received and email after signing up with a link to click on and proceed to the self serve set up process, did you receive this email? Check spam folder as well.
If not then you'll need to contact Koodo directly by sending them a Private message to their FB page or Twitter account where they can verify your account and send you a new email.
