I bought Nexus 5 with $20 micro-SIM card in Koodo Shop and I changed my original koodo standard SIM card to it, but how do I get the $20 back? I didn't see it in my balance.

The $20 you're speaking of is actually a first-time activation bonus for when you first open a prepaid account. Getting a replacement sim card or different sized sim card after the fact doesn't give you the bonus

Yes, you won't be receving the $20 bonus as its for first-time activations only

Could I return the standard SIM card back to them and get $20 + tax back? It is in 15 days.

No, sim cards are non-refundable. Only phones and accessories have the 15 day return policy.
